Redetermination Packet

Welcome to a faster, easier way to redetermine your eligibility for childcare assistance!

Please click on the appropriate redetermination package, print the documents, and mail, fax, email, or hand deliver to the Eligibility Specialist listed on the redetermination letter you recently received.
 
Package 1 – STANDARD
 
Package 2 – DISABILITY (If you are receiving services because you are permanently disabled.)
 
Package 3 – DCF REFERRAL (If you have an open DCF case with a referral.)
 
Package 4 – FUNDING (If you receive Relative Caregiver funds or TANF funds and do NOT have an open DCF case)
 
Once you have all your forms filled out completely, please email, fax, mail, or drop off in person to the Eligibility Specialist listed on the redetermination letter you received. Remember that all forms must be completed that apply to your case and that they all must be signed and dated as indicated. Failure to return the documents within the allowable time frame could result in a loss of eligibility.
 
If you are still eligible for services, your Specialist will contact you to set up a short appointment to come in and sign paperwork and pick up the child care certificate. If you are not eligible for services, your Specialist will contact you with an explanation and send you a Notice of Ineligibility.
