Once issued, vouchers are unable to be replaced!

Vouchers to be submitted with order form by Friday, June 1, 2012.

Absolutely NO late submissions will be accepted.

Any unused points submitted with your order will be null and void. For example, if your order total is 45 points and you submit 50 points, the remaining 5 points will be null and void.

Order form and vouchers to be mailed or dropped off to the ELC Rockledge Office. Rockledge Office (1018 S Florida Ave, Rockledge, FL 32955).

Only ONE order per provider will be excepted (NOT ONE PER CLASSROOM!)

Items are accepted "as is” and the Coalition is NOT responsible for damage that may occur to the item due to future use.

ELC reserves the right to substitute another item if the item requested is no longer available.

2. Place Your Order
All orders must be submitted by Friday, June 1, 2012. To place your order please use the Provider Store order form. Your order form must be mailed or hand delivered to the Rockledge office--NO FAXES! Only one order accepted per provider. VOUCHERS MUST BE SUBMITTED WITH YOUR ORDER FORM!
